Certificate Request

Many individuals request copies of their Baptismal record or other sacramental records for several reasons including Baptism Sponsorship, Confirmation Sponsorship, First Communion, Marriage, Religious Ed Registration, etc..

Sacramental records are located in the records from the parish at which the sacrament was celebrated. In addition, all recordable sacraments received by an individual are available at the parish in which he/she was BAPTIZED. All sacraments are posted in the Baptismal record books of the original parish of Baptism, even if currently a member of another parish, or confirmed or married in another parish.

Records will only be provided to the recipient, to the parents of a minor recipient, or to the requesting parish. Requests may be made in person, via phone, online, or by mail.

Sacramental Records Requests

The Parish Office will research the Sacrament Record Books and complete a Sacramental Certificate. When completed by the parish office, it is dated and signed by the current pastor and embossed with the parish seal. Completed certificates may be mailed to you or the parish requesting your Certificate, or you may pick it up from the parish office. You will be contacted when it is ready. Please allow 7 to 10 days for processing.

Sponsorship Certificate Requests

Individuals wishing to become a SPONSOR (Godparent) for a BAPTISM or CONFIRMATION are required to provide a Sponsorship Certificate TO the parish where the baptism or confirmation will take place. It must be FROM the parish they are currently attending.

Holy Trinity can only issue Sponsorship Certificates for those who are registered and active members of Holy Trinity in Webster, NY (no matter where the Baptism or Confirmation will take place).

Being a Sponsor is not just an honor given to a friend or relative. Being a Godparent or Sponsor implies a real appreciation and practice of the faith and a sense of responsibility towards the person being sponsored. Therefore, one should not be asked to be a sponsor simply because he/she is a relative or friend. Sponsors should be chosen because they are believers of the Catholic faith who KNOW, ACCEPT, PRACTICE, AND WITNESS to the faith.

To serve as a Godparent/Sponsor:

You must be at least 16 years of age; have already received the Sacraments of Initiation: ​Baptism, Confirmation and Eucharist; participate in Sunday Mass, Holy Days of Obligation and receive the Sacrament of Eucharist frequently; not be bound by any canonical penalty; and are not the father or mother of the one to be baptized or confirmed.
